Job Description

About the Role

We are seeking a committed and experienced Family Support Worker to join our team. The successful candidate will provide assessment, coordination, and goal-focused community access support for participants and their families. The role involves working collaboratively with families, plan nominees, support coordinators, and community service providers to ensure participants receive tailored support aligned with their NDIS goals and wellbeing needs.

Key Responsibilities

• Assess participants’ needs through interviews, functional observations and collaboration with families, plan nominees and support coordinators.
• Plan, develop and implement person-centred programs to enhance daily living skills, independence, and social/community participation.
• Monitor and report on participant progress, maintain case notes, and contribute to Care Plan reviews and quality improvement processes.
• Liaise with community organisations, welfare agencies, health providers, and government bodies to coordinate support and improve access to community resources.
• Assess community needs and identify appropriate services, programs and activities to assist participants in achieving their independence and wellbeing goals.
• Facilitate community participation activities, accompanying participants to appointments, recreational programs, workshops, and skill-building activities.
• Provide practical support to families to assist them in navigating services, strengthening capacity and promoting social inclusion.
• Prepare reports, documentation and contribute to internal organisational reporting, funding requirements and compliance obligations.
• Uphold safe work practices, incident reporting, and compliance with NDIS, WHS and organisational policies.

Skills & Experience Required:

• Minimum AQF Diploma in Community Services, Social Welfare, Human Services, Disability, Mental Health or related discipline, or equivalent experience.
• Demonstrated experience in family support, community services, disability support, casework or related field.
• Strong understanding of person-centred practice, community engagement, and family-focused support models.
• Excellent communication, documentation and liaison skills when working with participants, families, carers and external agencies.
• Ability to develop support plans, assess participant needs, and monitor progress against goals.
• Current Working With Vulnerable People (WWVP) check (or ability to obtain).
• Current National Police Check.
• Current Driver Licence and willingness to travel for participant/community visits

· Approved AFP
